Abstract
Keloids and hypertrophic scars results from excessive collagen deposition, the cause of which is not yet known. Unlike hypertrophic scars, keloids frequently persist at the site of injury, often recur after excision, and always overgrow the boundaries of the original wound.

There were many trials to control the abnormal scarring, but most of them were known to be unsatisfactory. The authors devised a new and supposedly better surgical method, and named ¡¯core extirpation¡¯, which is to remove the inner, underlaid collagen mass out of keloid and redrape the remained outer layer of Keloid onto the central denuded defect as a graft or a flap.

The authors treated 16 keloids from ears, trunk extremities, mons pubis, and labium majora by means of ¡¯core extirpation¡¯ in conjunction with D-penicillamine and colchicine p.o. for 1 month.

No recurrence was observed at all during clinical follow-up period of 3 months to 18 months. Partial flap or graft necroses were noted in only 3 large keloid of all the cases treated by our method.
